survey results update

I’m following up on my note from last week. Thank you to everyone who has filled out the survey so far. A few members mentioned they'd feel more comfortable responding anonymously, so I've removed the name and email fields to make that possible.

[The survey] should take about 4 minutes to complete, give or take. I am working on a project focused on the nonprofit sector and your survey feedback would be incredibly helpful. Thanks again ✌🏾
